Knee pain is rarely just a knee problem. The knee sits between two highly mobile joints — the hip above and the ankle below — and it depends on proper alignment and muscle balance throughout the lower kinetic chain to function without excessive stress. When the hip is weak, the foot pronates excessively, or the pelvis is misaligned, the forces transmitted through the knee change in ways that accelerate wear, increase inflammation, and contribute to pain.

Common mechanical causes of knee pain seen in our practice include:

Knee osteoarthritis involves the gradual breakdown of cartilage within the knee joint, leading to pain, stiffness, joint space narrowing, and reduced function. It is extremely common in adults over 50 and a leading reason patients are referred for knee replacement consultations — many of whom benefit significantly from conservative care before reaching that stage.

Patellofemoral pain syndrome describes pain at the front of the knee where the kneecap tracks over the femur. It is most common in active patients and those whose hip weakness or foot mechanics cause the kneecap to track unevenly, creating friction and irritation.

Iliotibial band syndrome involves tightness and inflammation of the IT band where it crosses the outer knee. It is particularly common in runners, cyclists, and patients who walk long distances, and is closely related to hip abductor weakness and poor pelvic stability.

Patellar tendinopathy involves degeneration of the patellar tendon below the kneecap. It typically presents as pain with jumping, squatting, or descending stairs and is a common source of chronic anterior knee pain in active patients.

Meniscus involvement can produce pain on the inner or outer joint line of the knee, often accompanied by catching, swelling, or limited range of motion. While significant meniscus tears may require surgical evaluation, many patients with minor meniscal involvement respond well to conservative care.

Pes anserine bursitis involves inflammation of the bursa on the inner side of the knee just below the joint. It is common in patients with knee osteoarthritis and those with tight hamstrings, and often presents as pain that is worse at night or when climbing stairs.
